Caboolture Centacare Services provides a community transport service for Home and Community Care clients and disadvanatged others (deemed eligible for transport) to:

  • Hospital / Medical appointments
  • Shopping
  • Social / Recreational activities

To receive a community transport service you must be registered as as client.

Please be aware that this is not an emergency service.

HOME MAINTENANCE AND HOME MODIFICATION (HACC) - Jointly Funded by the Federal and State Government

Caboolture Centacare Services HACC Home Maintenance and Home Modification is based on consumer needs.

This service will help you remain in your home environment as long as practical.

  • Through the installation of grab rails
  • Ramps
  • Hand held showers
  • Bath boards
  • Chair and bed raisers
  • Help with garden, yard maintenance & security
  • Handy work - home maintenance
  • Fixing taps
  • Toilet cisterns

You may be eligible for HACC services if you are a frail elderly person, a younger person with a disability and their carers.
VETERAN'S HOME CARE PROGRAM (V.H.C.)

Caboolture Centacare Services is a Service provider for Veteran's Home Care Program (V.H.C.)

We offer the following through DVA's veteran's Home care Program:

  • Domestic assistance
  • Personal care
  • Home and yard maintenance (limited to 15 hours per year)
  • Respite Care in home and emergency
COMMUNITY TRANSPORT'S FLEXI-SHOPPER

A pre-booked door to door service open to the general public in partnership with Caboolture Bus Lines travelling to the Morayfield Shopping Centre.

This sevice is pre-booked door to door and bookings must be made by 2.00pm the day before.

CABOOLTURE CENTACARE EDUCATIONAL AND RESPITE SERVICE

Clients who have suvvessfully obtained funding through either Post-School Services, Adult Lifestyle or other sources can access the following:

  • Individual programs
  • Age appropriate activities
  • Access to relevant TAFE courses
  • Community and recreational options
  • Work experience
  • Literacy, numeracy skill development
  • Acquisition of independent living skills
  • Participation in activities that promote self esteem eg. performing arts
  • Specialised assistance eg. speech therapy etc.

Clients who require respite / community access should phone the Program Coordinator on 5498 9154 to discuss their respite needs.

HOME ASSIST / SECURE - Funding by Housing Qld

Caboolture Centacare Services Home Assist Secure. Home Assist Secure will help you to:

  • Make more informed decisions about home maintenance, repairs, modifications and security
  • Feel safer living at home
  • Have easier access to and within you home, increase your confidence and about employing tradespeople
  • Incorporate home security routines into your daily activities
  • Subsidised assistance for home maintenance and minor modifications will be given if you are receiving a Commonwealth Govt pension or benefit.

Clients must be 60 years old or of any age with a disability, be living in their own home or private rental.

Donations over $2.00 to our services are Tax Deductible. These services receive government funding and they also rely on the Public for Donations for support so they can continue to deliver Quality Services.
